Third Party Administrator - Pharmacy Benefits Manager
Effective April 1, 2012, Third Party Administrators (TPAs) are required to submit initial applications electronically through Sircon at www.sircon.com/southdakota. Print a copy of the license application to retain for your own records; DO NOT mail it to the state. NO PAPER APPLICATIONS WILL BE ACCEPTED.
Please note that when submitting a TPA application, you must select the TPA License Application and TPA as the Qualification Type.
When submitting a Pharmacy Benefit Manager License you must select the TPA License Application and select the PBM Qualification Type.
If one or more of the background questions is answered "yes" and additional documentation is required, you must mail, fax, or e-mail it to:
